The Elephant in the Room: "Non-Compliance" is often not what it seems

 We keep calling it "patient non-compliance." But when you actually sit with families and hear what happened, it's almost never defiance — it's a person who didn't trust the instructions enough to follow them, and no one had time to find out why.
