Ongoing in-person nurse visits and care coordination within 30 miles of Raleigh, NC. Regular check-ins allow us to learn a client’s baseline health status and watch for early signs of decline. If something seems off, we notify family and/or their healthcare provider immediately.
Available weekly or every other week.
What’s included:
• Nurse visit at the client’s home or preferred location
• Verbal assessment, health monitoring, and assistance with vital signs (including weight check-especially important for heart patients)
• Short walk as appropriate to check balance, strength, and gait
• Medication check, refills called in when needed, and assistance filling pill boxes
• Coordination of appointments as needed
• Written visit report provided to family with client’s permission
*Accompaniment to medical appointments can be substituted for a home visit. A written report of the appointment will be provided to family afterwards with client’s permission.

Short-term nurse advocacy when things get complicated. Services are customized to your specific situation.

• Six hours of focused support for urgent or complex situations such as unexpected hospital admissions and difficult transitions of care
• Research, guidance on next steps and questions to ask, communication with providers or facilities, and coordination of care as needed
• Practical advocacy tailored to the immediate situation and concerns
