In Person Monitoring and Care Coordination

*Initial assessment required prior to or concurrently with the start of on-going monitoring and care coordination

Ongoing in-person nurse advocacy and care coordination visit within 30 miles of Raleigh, NC. Available weekly or every other week.
What’s included:
• Nurse visit at the client’s home or preferred location
• Verbal assessment, monitoring, and assistance with vital signs (including weight)
• Short walk as appropriate
• Medication check and assistance filling pill boxes
• Coordination of appointments as needed
• Written visit report provided to family (with client’s permission)
After purchase, we will contact you within one business day to confirm location, preferred schedule (weekly or every other week), and any specific needs.
Important: This is monitoring and care coordination, not hands on nursing care
